MY FAVORITE BACK TO SCHOOL IDEA IS...

Every year I try to collect data in a new and exciting way. My first year as an SLP I tried to collect work samples and use small sticker charts in a "Speech Folder" however it wasn't quite organized. Last year I used a SOAP note, but I couldn't keep up so I switched over to Super Duper Inc.'s data tracker app, but again I was doing all the work.

This year I am trying a combo of my new "Speech Portfolio" and the Super Duper Inc. Data Tracker. I want my students to be more responsible for their own tracking, especially the older students. Now I know this isn't a totally new idea, but I am trying to see if I find more success with this pack of printable sheets!

I bought folders with fasteners for .15 cents each at Wal-mart! All said and done only about $11.00 for 72 folders. Worth it for the fasteners alone.
First page-students can decorate to their liking. (P.S. Can you see my wedding flowers!?)
I start with an "About Me" page.  I think it's one of the best ways to get to know students and they can share as a group to get to know each other!
My version to share with my students!
At our school and many others we use "I can" or "I will" statements very often. I wanted to add a page so we can start off our speech sessions with a statement of positivity!
I like to make sure all my students know exactly WHY they are in therapy!

With my help, we write down goals and review them for a few weeks until memorized
I LOVE stickers-I usually give 1 for attendance and 1 for effort. However I included a homework chart for those of you that give homework!

I usually give a small prize every 15 stickers.
I want students to keep their own attendance this year, besides the attendance I take for my billing! This might be my favorite page, this is automatic proof for teachers, administrators and parents that students have been in my office. Love it so much, I made this page FREE!!!! Click here!
As I collect data on the tracker app, at the end of the session I have students color in their bar graphs for each goal we worked on. This is a great visual.  I use this in conjunction with my Objective Binder so students know what it means to "achieve a goal!"
I also like to keep my own notes. I typically keep track of what we did each day, behavior, more detailed notes on progress etc! Then I add it to the students personal file when the page is full or at IEP time.

I also add extra work that students complete during the year, art projects, additional notes pages and such!

  3. Love your packet....my favorite page has got to be the positive affirmations! I LOVE that! When your students get a little down they can get rid of that stinkin' thinkin' by reading those statements! Perfect!

  4. Very cute idea for the folder! If you decide to do this idea again next school year, watch Staples and Office Depot sale ads (in July and Aug) -- they will have those same pocket folders for 0.01 each! Honest! (Check out this week's Office Depot ad) Office Depot will allow you to triple the limit in the ad when you show your school ID. -LyndaSLP123

  6. What a great idea! I think students make so much more progress when they know what they are working on and are invested in the process. These folders will definitely help!
